Terms of Promotions
- The qualifying New Patient will receive a New Patient Exam and Necessary X-rays or Emergency New Patients will qualify if recommended treatment is performed
- Comprehensive Exam (150)- includes 4 Bitewing X-Rays (274)
- Recipients must be at least 18 years old
- Any decay will need to be treated prior to whitening kit being released due to the fact that teeth with decay may not be whitened safely
- Limit 1 per family
- The Patient and any other person responsible for payment has the right to refuse to pay, cancel payment, or be reimbursed for any other service, examination, or treatment which is performed as a result of and within 72 hours of responding to the advertisement for the free, discounted, or reduced fee service, examination or treatment.
